Objective: To evaluate the safety and outcome of early appendectomy in appendicular mass especially in the era of laparoscopy. Patients and methods: A prospective study of 39 patients who were operated for appendicular mass in the period from September 2005 to October 2009. Early appendectomy was performed through open (OA) or laparoscopic techniques (LA). Results: 21(53.8%) females and 18 (46.2%) males were included. The mean age was 27.9 years (range 6-65). OA was done in 32(82%) patients and LA in 7(18%). The appendix could not be removed laparoscopically only in one patient. The mean operative time was 75 min (range 40-100)in OA and 97 min (range 85-140) in LA. No intraoperative complications and no mortality. Postoperative complications occured in 6 (15.3%) patients; 5 superficial wound infection and one severe port-site infection. The length of hospital stay ranged from 3-7 days.Conclusion: Immediate appendectomy in appendicular mass is feasible and can be performed safely through open or lap techniques. It eliminates the risk of complications of appendicular mass, recurrent appendicitis and the need for re-admission for interval appendectomy. Surgical intervention for appendicular mass should only be done by a senior surgeon using careful and meticulous techniques.
