Objective: Evaluation of the outcome of treating 2-part nonunion of proximal humeral fracture with large set 4.5 mm T-locked plate.

Patients and Methods: From January 2004 to July 2007, twelve patients with 2-part proximal humeral nonunion underwent fixation with large set 4.5 mm T-locked plate in The Department of Orthopedics & Traumatology, Sohag University Hospital. The mean time interval from trauma to surgery for nonunion was 10.5 months, ranging from 6-18 months.

Results: The mean follow-up period was 20 months (range, 14-36 months). Eleven patients (91.7%) achieved osseous union at mean of 5.8 months after treatment and had good functions of the shoulder joint and the upper extremity, while one patient did not achieved osseous union up to 8 months follow up and was referred for another center for shoulder arthroplasty. No patient experienced any permanent neurological deficit.

Conclusion; Large set 4.5 mm T-locked plate is an effective alternative treatment for nonunion of proximal humeral fracture.
